Taxi drivers must complete a Taxi medical assessment to apply for a Taxi licence or to renew their licences.


We have been approved by a number of councils to provide Taxi medicals.


The examination will be carried out in accordance with the latest requirements of DVLA Medical D4 form.

If your council is not mentioned below , please give us a call or check with the council.

  • Some councils require full Medical Record or a medical summary to be reviewed by the doctor signing your D4 form so please contact your us prior to booking your taxi medical.

The following councils do not require a medical summary from your GP
