Who needs to complete a D4 DVLA driver medical assessment?A doctor will need to complete this form if you are applying for a lorry or bus driving licence. They can be completed by any GMC approved Doctor. The Medical D4 form will also need to be completed and send to your local council if you are applying for a Taxi License. Some councils require a medical summary from your GP. Please check your requirement with your local council, alternatively ask us in the chat about your council or give us a call on 07415788851.

What is your cancelation policy?We can understand if your schedule has changed or you need to cancel your booking. You will get a full refund if you give us notice more than 48 hours before your clinic appointment. Please get in touch via the chat service or give us a call. We can then cancel or reschedule your appointment. We may also be able change it to a different location of your choice.

How quickly can I get an appointment?We always try to see drivers as quickly as possible in a place convenient to you but our clinics are in high demand so please get in touch as soon as you can to secure your appointment. Call us on, email or book online and we will schedule your appointment at your closest centre(s), including on evenings and weekends.

Who will do my D4 medical assessment?Your D4 assessment will be completed by one of our GMC registered doctors in accordance with DVLA requirements.

How can I pay for my medical assessment?You can book and pay through our website. Arrange to pay on the day of your appointment. This can be done with card we accept CASH, VISA, MASTERCARD and CREDIT CARD. There is 2.5% card transaction fee if you pay by card at the location, there is no fee for bank transfers or when you make the payment online during your booking.

What can I expect at my Medical D4 appointment?During the medical our Doctor will: Complete an Eye test using the Snellen Chart ( an eyesight testing chart) Blood Pressure Reading and Recording. Record any medication you’re currently taking Ask about your previous medical history You may be asked to provide a urine sample, which we use to test for diabetes.

What happens with the completed D4 form ?The D4 form is the medical aspect of your licence application. This needs to be posted to DVLA alongside any other forms that you may need to fill such as the D2 or D47P form. If you fail your Medical, you will be offered a 4 month free retest. This is only valid if you return within 4 months of your appointment date.

Will the doctor help me complete the D9, D2 or D47P form?Our doctor will complete the D4 medical for you but you will need to complete the D2 or D47P renewal form yourself. For more information on how to complete your D9, D2 or D47P form please visit the DVLA website. You can find D2 forms at some post offices or local HGV/PVC training schools. If you need a D47P renewal form, visit the DLVA website and they will send you a pack by post.

What do I need to bring to my D4 appointment?Some local councils require full medical records from your GP. If you are unsure if your council requires this, please check with us. Below is a list of the five items you will need to bring to your Medical D4 assessment. 1) D4 Medical form/ taxi medical form. 2) Driving licence (and/or passport) 3) Your driving glasses and optical prescription (contact lenses must be removed for the ‘uncorrected vision’ eye test) 4) A list of all your current medication, the doses and the date you began taking them 5) Any hospital letters or details of hospital consultants who are treating you

How long does the medical assessment take?The D4 medical assessments can take around 10-20 minutes but they can be longer depending on your medical history.

How does your Do you offer a Discount Code?We offer a £5 discount code for our Partners. The driver should apply this at the time of the booking online. Discounted payment will then be taken at the location. The discount will only be awarded if you have booked online using the discount code. The discount code cannot be used for telephone bookings.

I am a Taxi driver and I forgot to bring a Medical Summary or relevant medicals forms to my appointment?It is your responsibility to bring your medical summary and medical forms. The Medical summary is required only for Taxi drivers and only for some councils please double check with us or your council if you don't know what is required. Medical summary is not required for Lorry/Bus drivers.
