Service Description
Obtain your complete HGV/LGV Medical for only £50 including VAT. Our GMC approved medical doctors will complete the Medical Examination DVLA D4 form. Perform an examination which includes : 1) Eye test 2) Blood pressure recording 3) A medical history check We do not not perform a Urine, Drug or Blood test as part of our assessment for HGV drivers. Anyone over the age of 18 must have a medical assessment and complete a DVLA D4 form when first applying for a HGV/LGV licence. A further medical assessment is required at age of 45 and every 5 years there after until reaching the age of 65. DVLA must be made aware of any changes to your health as a HGV driver at any time. You will need to take with you. 1) Proof of ID (ideally driving licence) 2) If you have seen a doctor recently, any letters from your doctor or hospital 3) A list of medications if you are taking any, the doses and the date you began taking them. 4) Your driving glasses and optician prescription (be able to remove your contact lenses for part of the Eye test) 5) D4 Medical form/ (or form relating to the medical you have booked). Our doctors may have spares for this. Always better if you bring your own.
